When you are coming to apply please bring the following things with you:

• Certified copy of applicant's Birth Certificate or Original Naturalization Certificate if they were not born in the US.
• Two Passport Photos. These need to be specific passport photos, please visit the Passport Agency Website for details.
• Driver's License or State ID. If applicant is under 16 years of age, we require both Parents Identification. If applicant is older than 16 but younger than 18 we will require one parent ID, and one from applicant. NOTE: State Identification card must have picture and be current, no expired ID's will be accepted.

Please have application filled out completely and Not Signed, we have to WITNESS your signature on the application.

Monday Thru Friday

 Passport Book Fees:

16 and older - $100 ( $25 for Clerk's Processing Fee, $75 for Passport)

15 and younger - $85 ( $25 for Clerk's Processing Fee, $75 for Passport)

Passport Card Fees:

16 and Older - $45 ( $25 for Clerk's Processing Fee, $20 for Card)

15 and younger - $35 ( $ 25 for Clerk's Processing Fee, $10 for Card)

Forms to be used:

First Passport Application, or expired passport is over 15 years since first issued, or current or expired passport was issued when applicant was under 16 years of age.

                                 Form DS-11

Renewing expired passport that was issued to applicant at age 16 or older, and passport was issued 15 years or less.

                                 Form DS-82

All persons applying for passport must appear in person, if applicant is under 16 years of age, both parents are required. if applicant is older than 16 but younger than18 only one parent is required with applicant.

if one parent cannot be present, the parent who cannot attend must have a notarized Parental Consent Affidavit in place of the parent. you can find this form on the passport agency website.

Payments:

if you are paying with cash, Please bring exact amount.

if you are paying with check or money order, Please bring two checks/money orders or one check/money order and cash( $25 for the clerk's fee).

The fees are a two part fee.
