Children & Youth with Special Health Care Needs
The Children and Youth with Special Health Care Needs (CYSHCN) program at Okanogan County Public Health District (OCPHD) serves children and youth, birth to 18 years of age who have, or who are at risk of developing, serious chronic physical, developmental, behavioral, or emotional conditions.
Program staff assist families in learning about and connecting with community organizations and services, support groups, health, financial and educational resources regarding the child’s medical condition and special needs.
Eligibility
This service is provided to families regardless of income and there is no charge to the family for CYSHCN services.
An individual parent or guardian, health care providers, hospital representatives and relevant community organizations can send a CYSHCN services referral by completing the online form
Examples for a CYSHCN Public Health Referral
A child who has a complex health need and the family needs assistance with:
- Connecting to early intervention, therapists or specialized services
- Coordinating medical or behavioral health providers
- Accessing local resources and/or services for a child
- Finding financial assistance for a child’s health-related need
- Understanding and/or coping with a child’s chronic health condition
